PP77 Safety, Effectiveness, And Cost Effectiveness Of Telemedicine In Neurological Diseases

Introduction
Telemedicine has been introduced in health services, but uncertainties about the real value of this strategy in the management of neurological diseases remain.
MethodsA systematic review was undertaken of available scientific literature on the safety, effectiveness, and cost effectiveness of telemedicine combined with in-person visits, compared with usual care, for the treatment and follow-up assessment of patients with neurological diseases. The overall effect size for each neurological disease was estimated using meta-analysis. An economic analysis was performed from the societal and Spanish healthcare system perspectives.
ResultsTwo economic studies were included for cost effectiveness and 25 randomized controlled trails (n=8,976 patients) were included for the effectiveness and safety assessment (11 on cerebrovascular diseases, four on Parkinson’s disease, three on multiple sclerosis, two on epilepsy, and one each on brain damage, dementia, spina bifida, migraine, and cerebral palsy). The types of telemedicine evaluated included: virtual visits (11 studies); telerehabilitation (seven studies); telephone calls (three studies); smartphone apps (two studies); and online software for computers (two studies). Subgroup analysis by type of telemedicine indicated no discernible effect for telemedicine combined with in-person visits on most of the outcomes analyzed for the various neurological diseases. Given the heterogeneity of diseases, types of telemedicine, and the results observed, a cost-minimization analysis was conducted. Combining telemedicine with in-person visits would cost EUR 2.55 per patient from the perspective of the healthcare system, but it would result in cost savings (EUR 27.34 per patient) from the societal perspective.
ConclusionsThe safety and effectiveness of combining in-person visits with telemedicine was similar to that of usual care, but it could be a cost-saving strategy in Spain from a societal perspective.

The role of morphemes in lexical recognition has been extensively explored in recent years, although the evidence from older adults is extremely scarce. In this study, we carry out a lexical decision task to assess the interference generated by morphological composition of pseudo-words (i.e., the longer and more error prone decisions on pseudo-words made up of morphemes in comparison to pseudo-words without morphological appearance) in a group of young and older adults (mean = 74 years). The results show the expected effect on both response latencies and error rates for both groups. The effect of imageability is also significant. The specific results for the older adults show an interaction between the morphological effect and cognitive reserve: older adults with higher levels of cognitive reserve are more sensitive to morphological interference than older adults with lower cognitive reserve. The overall results are interpreted based on current models of morphological processing and aging.

Healthcare workers (HCWs) not fulfilling the coronavirus disease 2019 (COVID-19) case definition underwent severe acute respiratory coronavirus virus 2 (SARS-CoV-2) screening. Risk of exposure, adherence to personal protective equipment (PPE), and symptoms were assessed. In total, 2,000 HCWs were screened: 5.5% were positive for SARS-CoV-2 by polymerase chain reaction (PCR). There were no differences in PPE use between SARS-CoV-2–positive and –negative HCWs (adherence, >90%). Nursing and kitchen staff were independently associated with positive SARS-CoV-2 results.
How Do Parents Influence Child Disruptive Behavior After Acquired Brain Injury? Evidence From a Mediation Model and Path Analysis

Objectives: Children with acquired brain injury (ABI) can present with disruptive behavior, which is often a consequence of injury and parent factors. Parent factors are associated with child disruptive behavior. Furthermore, disinhibition in the child also leads to disruptive behavior. However, it is unclear how these factors interact. We investigated whether parental factors influence child disruptive behavior following ABI and how these factors interact. Methods: Parents of 77 children with ABI participated in the study. Parent factors (executive dysfunction, trait-anxiety), potential intervention targets (dysfunctional parenting practices, parental stress, child disinhibition), and child disruptive behavior were assessed. A hypothetical model based on the literature was tested using mediation and path analysis. Results: Mediation analysis revealed that child disinhibition and dysfunctional parenting practices mediated the association of parent factors and child disruptive behavior. Parents’ executive dysfunction mediated the association of dysfunctional parenting practices, parental stress and parent trait-anxiety. Parenting practices mediated the association of executive dysfunction and child disruptive behavior. Path analysis indices indicated good model adjustment. Comparative and Tucker-Lewis Index were >0.95, and the root mean square error of approximation was 0.059, with a chi-square of 0.25. Conclusions: A low level of parental trait-anxiety may be required to reduce dysfunctional parenting practices and child disinhibition. Impairments in child disinhibition can be exacerbated when parents present with high trait-anxiety. Child disinhibition is the major contributor of disruptive behavior reported by parents and teachers. The current study provides evidence of parent anxiety and child disinhibition as possible modifiable intervention targets for reducing child disruptive behavior. (JINS, 2019, 25, 237–248)

Building materials are very complex samples of worldwide importance; hence quantitative knowledge of their mineralogical composition is necessary to predict performances. Rietveld quantitative phase analysis (RQPA) allows a direct measurement of the crystalline phase contents of cements. We highlight in this paper the use of laboratory X-ray powder diffraction (LXRPD) employing high-energy radiation, molybdenum (Mo), for attaining the RQPA of cements. Firstly, we evaluate the accuracy of RQPA employing a commercial calcium sulfoaluminate clinker with gypsum. In addition to MoKα1 and MoKα1,2 radiations, Cu and synchrotron patterns are also analyzed for the sake of comparison. Secondly, the assessment of the accuracy of RQPA results obtained using different radiations (synchrotron, Mo, and Cu) and geometries (reflection and transmission) is performed by analyzing two well-known commercial samples. As expected, for LXRPD data, accuracy in the RQPA results improves as the irradiated volume increases. Finally, three very complex aged hydrated cements have been analyzed using MoKα1-LXRPD and Synchrotron-XRPD. The main overall outcome of this work is the benefit for RQPA of using strictly monochromatic MoKα1 radiation. Best laboratory results arise from MoKα1 data as the effective tested volume is much increased but peak overlapping is not swelled.
Powder diffraction analysis of gemstone inclusions

Gemstones are pieces of materials that once cut and polished are used as jewels or adornments. Gemstones may be single crystal (such as diamonds), polycrystalline (such as lapis lazuli), or amorphous (such as amber). In any case, gems may have inclusions that may yield a variety of optic effects. It is also important to unravel the crystal structure of the inclusion(s) in order to determine the origin of the gem and to help to understand their formation mechanism. Here, we expand the use of powder diffraction to identify crystalline inclusions in bulk gemstones highlighting Mo Kα radiation to penetrate within compact gems. Initially, rock crystal quartz with rutile needles was investigated and rutile diffraction peaks were more conspicuous in the Mo pattern than in the Cu pattern. Next, rock crystal quartz with beetle legs was characterized and the red iron oxide inclusion was identified as hematite. The study of a fake gem, glass showing aventurine effect, gave the diffraction peaks of metallic copper. Later, polycrystalline gems, moss agate, and aventurine quartz were also studied. The powder patterns of these compact gemstones could be successfully fitted using the Rietveld method. Finally, we discuss opportunities for further improvements in laboratory powder diffraction to characterize inclusions in compact gems.
